Forms that capture leads, not just data
Start from a ready-made template, arrange it in the designer, and embed it anywhere. Every submit becomes a lead in your pipeline, with the campaign that brought it attached and, if you want, an AI voice call back within seconds.
A working form in three steps
- 1
Pick a template
Contact, appointment, quote, sign-up, support and more. Each one opens as a finished, designed form in your own language, not a stack of bare inputs.
- 2
Make it yours
Add, remove and reorder fields, set labels, options and placeholders, and adjust spacing, colors and the button until the form looks like your brand.
- 3
Put it to work
Copy the embed code onto your own site, or drop the form into a TalkOmni landing page. Both feed the same pipeline.
What the form designer gives you
Ready-made templates
Every template opens with its fields, copy and button already designed, in your own language, so you start from something finished instead of an empty canvas.
Visual designer
Text, email, phone, long text, dropdown, checkbox and consent fields, laid out in one or two columns with the spacing, colors and typography you choose.
Embed it anywhere
One snippet puts the form on your own site, and you can list exactly which origins are allowed to load it.
Voice call back on submit
Turn the voice toggle on and the AI voice agent calls the lead back moments after they submit, in their language, grounded in your knowledge.
Consent is built in
A consent field ships with the template and is stored with the submission. No outbound call is ever made without it, which is what KVKK and IYS require.
It knows where the lead came from
utm_source, utm_campaign, gclid, fbclid, the referrer and the landing URL are captured at submit and stay attached to the lead for good.
Pushed to your systems
Every capture fires a lead.captured webhook to your backend with the attribution included, so your own CRM can be the system of record.
Already have your own form?
Keep it. When it is submitted, your backend posts to our API and the same pipeline runs: the lead is captured, the webhook fires and the voice agent calls back. Our form becomes just the definition, never rendered.
The assistant captures leads too
In a chat or on a call, the assistant can take the same contact details with its built-in lead action, so conversations and form fills land in one place.
Frequently asked questions
- Do I need a developer?
- No. You pick a template, edit it in the designer and paste one snippet into your site. There is no code to write and no form service to sign up for separately.
- Where can I use the form?
- On your own website through the embed code, or inside a TalkOmni landing page built in the same wizard. Both send to the same pipeline.
- I already have a form on my site. Can I keep it?
- Yes. Define the fields once in TalkOmni without ever rendering our form, then have your backend post to POST /v1/projects/{id}/forms/{formId}/submissions when your own form is submitted. You get the lead, the lead.captured webhook and the consent-gated voice call back exactly as if the form were ours. The endpoint needs an API key with the forms:write scope, which is part of the API access entitlement.
- Can the AI call the person who filled it in?
- Yes. With the voice toggle on, the voice agent calls the lead back after they submit, as long as they gave consent on the form.
- How is consent handled?
- The consent checkbox is part of the form and its text is yours to write. It is stored with the submission, and no outbound call is made without it.
- Can I tell which ad brought the lead?
- Yes. Tag your ad links with utm_source and utm_campaign and the source is captured at submit, shown in the leads report and included in the webhook.
- Can I get the submissions out?
- Yes. Submissions are listed in the panel, leads export to CSV, and every capture also fires a webhook to your own backend.
