Build it in the form designer
Start from the pre-session template or a blank page, split it with page breaks, add rules that open a section only for the answers that need it, and a table for answers that run to many rows.

Let bookings send it
Tick "send after booking" and every appointment brings the customer a personal link on the channel they booked from, and by e-mail when you have their address. Or share a public link, a QR code or an embed on your own site.

Read the answers next to the booking
Responses list whose they are, where they stand and when they were completed. Open one right beside the appointment, and let the retention window delete it when its time comes.
What makes a survey different from a form
Sealed, not just stored
Answers are encrypted with their own key. Project admins and agents see only whether a survey was filled in, never what it says, and answers are never exported to Excel.
The assistant cannot read them
No tool, no search and no summary mail reaches a survey answer. Questions about health, family or private life stay between the person and you.
Consent before the first question
The person accepts your consent text before they see a single question, and which version they accepted is stored with the answers.
Saved page by page
A long questionnaire does not have to be finished in one sitting. Every page is saved, and the same link opens where the person stopped.
The right survey for each booking
A question on your booking form, such as who the session is for, decides which survey goes out, so a parent booking for a child never gets the adult questionnaire.
Deleted on schedule
Answers are deleted a set number of days after the appointment, 90 by default, and an erasure request removes them across every project at once.
Frequently asked questions
- Who can read the answers?
- Only the company owner and company admins. Project admins and agents see the survey's state on the appointment, not its answers, and the assistant cannot read them at all.
- Does a survey create a lead?
- No, and that is the difference between the two. A form becomes a lead your team works; a survey stays sealed. Use a form when you want a lead.
- How does the customer get the link?
- With "send after booking" on, every booking made by the assistant, a booking form, the panel or the API sends a personal link on the customer's channel, and by e-mail when their address is known and the project has a sending account. The link works until the day after the appointment.
- Can people fill it in without an appointment?
- Yes. Give the survey a public address and share it as a link, a QR code or an embed on your own site. Someone who closes the page and comes back on the same device continues where they stopped.
- Which plan includes surveys?
- Every plan. Basic keeps 3 surveys active at once, Pro 15 and Enterprise 50, and each completed survey counts once against the monthly form submissions.
